The schematic and PCB were made using two parts unavailable in the standard EAGLE libraries: a schematic frame from the freely available SparkFun library (http://www.sparkfun.com/) and a Gilberti Industries logo from the GilbertiIndustries library.  Both libraries are included on the off chance that not having them prevents opening and using the CAD files.

The bill of materials only includes parts needed to populate the board and their corresponding PCB part identifying codes.  Additional support parts can be found in the original article in MAKE Magazine, and include, but are not limited to, the 120V to -12.6V/0V/+12.6V 2A transformer, input sound jacks, and speaker connections.

This design uses a pair of 100KΩ resistors to fix the output volume on both channels, making the input device responsible for volume control.  These resistors can be replaced with a single 100kΩ dual potentiometer as per the instructions originally presented in MAKE Magazine.
